Designate someone to keep the minutes and a list of "action items" for follow up. 

Team input via regular meetings

Many companies also have a weekly or bi-weekly meeting to track progress and update the list. Although this sounds simplistic, it can help by breaking up seemingly insurmountable tasks into smaller pieces.
